Dear Parents,
We are so glad to provide movement and music to your child each week at your child’s school. The Dance Foundation is a non-profit organization whose mission is to teach the art of dance to all, inspiring creativity and confidence.
We request your consent of The Dance Foundation’s use of photos and/or video taken of your child participating in our program. The photos/video are used as a part of our grant proposals, donor request letters, and various other fundraising and marketing materials.
After you complete the form below, we will send you a link and password to a page with details, updates, and photos from our classes at your child’s school!

By clicking "submit" below, you give your consent for the use of your child’s photograph or video image or likeness by The Dance Foundation for marketing and fundraising purposes, with the assurance that your child’s name will not be associated commercially with the photograph, so as to provide you with anonymity, and you waive all claims for any compensation for such use or for damages.
